Q: What
do I need to bring to my appointment? |
After
you contact us to
schedule your initial meeting, we will send you an appointment
confirmation letter that will include a family information sheet
and a checklist of items you should bring with you.
Please
try to fill out the information sheet in advance of your
appointment. But, if
you can't find or gather all the information we request before your appointment,
that's okay. Don't postpone your appointment. Bring what you can
locate. We should be able to get the remaining paperwork we
need.
