• Finding ways to keep the client at home, or finding the best care facility if needed

• Screening, arranging, and monitoring in-home help or other services

• Bill paying and financial management

• Selecting a nursing home or assisted living facility & application procedures

• Serving as a liaison for families that can't be close by; making sure things are going well and alerting families to problems
